The top text section and thank you image to the right could be changed to speak to the person filling in their information as the referral, rather than assuming the current customer is filling out this form with their friends info.  

Below this I've populated the fields based on the query string in the URL which can be set by the QR code.

I'm only displaying it here to show it working, on the live site we should hide this data so it is not easily edited or confused, but still have it submitted with the form. 

To the referrer filling out the form, they would only see a sentence like the one above which calls out the referrer by first name,  for a little extra personalization. 

This form already adds the referral tag to the new users, and puts the referred persons email in the referrers account under "Referrals". 

You currently have 7 people tagged as referrals in the system (Fluent CRM) but I think those are all tests. 

I have not setup a google sheet for this, but it would be easy to have these go to a google sheet with the referrers and referrals all listed.
